CommHIT Everywhere ensures distance no longer stands between people and the care they deserve.
What is CommHIT Everywhere
CommHIT Everywhere is a practical infrastructure of care built to give every community a clear path to better health.
Mobile screening vans stop routinely at rural community pharmacies, offer screenings and connecting patients to immediate local care close to home.
EMS agencies bring non‑emergency care into patients’ homes and help them stay as well as they can.
FDA‑cleared devices are placed in patients’ homes to alert their care team when a higher level of care is needed.
Pharmacies offer vitals checks, medication reconciliation, and on‑demand virtual visits with easy follow‑up care.
Extends the Florida Health Information Exchange into rural regions so appropriate care teams see the same patient story.
The rollout
Each phase spells out what changes for families, so nobody has to guess when care arrives in their county.
What else is on the horizon
New ways to deliver care and support, tested in our communities as CommHIT Everywhere grows.
Counties and Super Regions
Counties are grouped into Super Regions, each with its own mobile van route. Schedules are being finalized.
